CONSULTANT TERMS OF REFERENCE REVIEW OF PRESERVICE NURSING AND MIDWIFERY CURRICULA: MINISTRY OF HEALTH AND CHILD CARE – NURSING SERVICES DIRECTORATE AND AIDS & TB PROGRAMM

To support the review of Preservice nurses training curriculum with a focus on HIV PREVENTION services and inclusion of VMMC.

DURATION-
Preservice: 45 days
Post Basic: 15 days.

The Zimbabwe Ministry of Health and Child Care (MoHCC), with support from its partners PSH, PSI and ZimTECH (ZAZIC consortium) has been implementing Voluntary Medical Male Circumcision (VMMC) as an additional HIV prevention strategy since 2009. The first decade of VMMC implementation was a vertical approach to allow for rapid scale up "catch-up" phase where sexually active males were prioritized to enable immediate and near-term reduction in HIV incidence. The program is now transitioning to "sustainability" phase that aims to move toward routine offering of VMMC to adolescents to maintain the long-term impact of circumcision on HIV incidence. The MOHCC developed a three-year strategic plan named the Sustainability Transition Implementation Plan (STIP) 2019 – 2021 for VMMC. The STIP strategic plan defined VMMC program sustainability as: “The managerial, financial, and operational ability to deliver and maintain 80% Voluntary Medical Male Circumcision coverage to ensure long-term health benefits and reduction in new HIV infections. This will be achieved through conformity to social norms, local ownership rendering the program affordable, accessible and acceptable to all.” One of the key strategic objectives meant to increase program accessibility is the emphasis on efficient decentralization of VMMC services in order to meet both scale-up and maintenance phase targets”.

The current vertical VMMC program has been supporting the MOHCC with training of doctors and nurses to provide VMMC services at provincial, district and mission hospitals. A strategy meant to increase human resource capacity to provide quality VMMC services with an increased focus on broadening the scope of practice of nurses at different levels. The STIP goal of decentralizing service provision to 80% of primary health facilities the program has struggled to keep pace with the volume of potential trainees who require training and certification in the shortest possible time in order to translate trainings to service delivery and reaching set coverages. MoHCC AIDs and TB unit proposes the inclusion of VMMC into preservice training for nurses with the first batch of trainees graduating by the end of 2023.

The HIV prevention programme within the AIDS and TB unit in the Ministry of Health and Child Care intends to build and strengthen the human resource capacity of the health system in order to provide a comprehensive package for combination HIV prevention services. Combination HIV prevention in Zimbabwe comprises of HIV Testing Services; Voluntary Medical Male Circumcision; Condom and STI prevention; and Pre-Exposure Prophylaxis; and the Key populations program. PrEP and the KP programmes being relatively new within the HIV prevention space and not currently included in Preservice curriculum. In view of this MOHCC seeks to conduct a comprehensive landscape analysis of its combination HIV prevention preservice training needs for optimal service delivery. The ultimate result of the review and updating processes of the training curriculum will produce a competent graduate, skilful in offering the full package of HIV prevention services.
It has been noted that the Registered General Nurse (RGN) pre-service curriculum was last updated in 2011. In view of this, a lot of changes, innovations, and new information have taken place and has not been incorporated into the curriculum. VMMC and some of HIV prevention components were included during curriculum review in 2011 but without the practical components included. While the nurses have generally good appreciation of VMMC and other HIV prevention programs during the pre-service period of instruction, but they are not equipped with practical skills for HIV testing as well as VMMC surgical skills to deliver quality services on completion of training. In view of this, the programs has invested significant resources to in service capacity building of service providers to enable them deliver high quality services. Providing necessary practical skills on some of the HIV prevention programs during pre-service training is however the most cost effective and more sustainable way of producing technically competent providers.

In view of this there is need for the process of reviewing the preservice curriculum to be focused on those technical and practical areas needing additions and or modifications. It is against this background that a technical consultant be sorted to lead this process. The process will not be confined to VMMC and HIV Biomedical Prevention but rather cover all aspects of the nurses’ curriculum. These Terms of Reference will focus on the roles and responsibilities of the consultants for achieving the goal of developing the requisite preservice curriculum.

Duties and Responsibilities

Purpose of the Consultancy

To lead the MoHCC and its partners in the process of reviewing of all components of the nurse's curriculum including VMMC and HIV Biomedical prevention modules (PREP, STI, Condoms and Key Populations) . Key responsibilities include leading the technical working group in reviewing the curriculum, development of teaching aids, facilitators manual, learners guide and learning assessment materials to be used by nurse tutors and clinical instructors who are responsible for practical aspects of the nurses’ training.

Key Deliverable
The following is the summary of the key deliverables of the VMMC Preservice training consultant.
Objective 1 - To develop the road map for the curriculum development process.
Activity - a. Create the curriculum development technical working group
b. Conduct a 3-day inception and consultative meeting with key stakeholders
c. Conduct a needs assessment and interviews with Nursing school authorities to understand successes and challenges
d. Develop a workplan to be followed throughout the curriculum development process
Output -
• Insights on the gaps
• List of proposed stakeholders
• Workplan
Objective 2 - To conduct curriculum development workshops.
Activity- a. Conduct curriculum review workshops
b. Capture all the meeting deliberations and recommendations.
c. Incorporate
comments and submit documents to MoHCC
Output - Revised curriculum content
Objective 3 - Facilitate development of teaching and learning materials.
Activity - a. Develop teaching, learning assessment materials.
b. Develop competency assessment tools for HIV prevention programs for use during preservice training.
Output - • Teaching aids, guides for learners, manuals for tutors and assessment tools.
Objective 4 - To document process and lessons learnt during the process.
Activity- a. Lead the team to document the process and lessons learnt for sharing nationally and globally.
Output -
• Documentation of the inclusion of VMMC module into Nurses Preservice training module
• Documentation of the implementation process
Objective 5 - Report writing
Activity - a. Write a consolidated report for all the activities.
Output - Final Report.

The exercise will be conducted with full logistical support from the MoHCC, PSH and CHAI. The consultant will work under the direction of the Director of Nursing Services with support from the Directorate AIDS and STI and TB programmes.

Payment
Payment will be based on the rates agreed between MoHCC/PSH/CHAI and the selected consultant and in accordance with partner rules and procedures. Terms and conditions of payment will be outlined in the contract for a total of 45 working days for preservice and 15 days post basic courses.

Qualifications and Experience

• At least 10 years of demonstrated successful experience in MoHCC nursing preservice training.
• Experience working with MoHCC Nursing Directorate
• Ability to pull together multiple insights into a common point of view that can be distilled into something simple for stakeholder consumption.
• Experience in training design, delivery, and facilitation
• Good writing and communication skills
• Familiarity with the Nurse or Midwifery education system in Zimbabwe as well global trends in nurse practice

Qualification
A minimum of a Master’s degree majoring in Education, Nursing or Midwifery Education
