Team Leader: Faith Community Initiative (FCI Community)

Reports to: Directly to Provincial Technical Officer, Provincial SI Officer

HIV Prevention, Care and Treatment Support - Faith Community Initiative (HPCTS-FCI). The Team Leader of the FCI Community Post is a nurse with overall oversight of the community post. She/ He is responsible for the smooth running of the community post and is responsible for the onsite staff and FCI champions. She /He plans, organises, monitors and evaluates the activities of the site to reach the target populations; men, children and adolescents.

Duties and Responsibilities

Responsible for the overall day-to-day running of the community post including coordinating, support and supervision of the staff attached to the community post; data clerk, counsellor, FCI champions and driver; Planning for the site routine HIV prevention, care and treatment support activities; Ensure clients visiting the community post and those being attended to in the community are well served, taking into accounts all the principles of customer care; Ensures there is adherence to targeted approaches to reach men, adolescents and children; Keep a complete and accurate inventory of medicines, HTS consumables and supplies at the community post, monitor and supervise their usage and in liaison with the ZACH logistics unit and the parent facility Sister-In-Charge, ensuring that there are no stock outs; Ensures adequate stocks of referral slips, community post supplies and infection prevention and control utilities; Clinical assessment and management of PLHIV (new and old clients); Antiretroviral therapy, Screening and prevention of specific OIs including TB, Reproductive Health Services (syndromic management of STIs), GBV; Provide ARVs for pre and post-exposure prophylaxis (PEP); Provision of TB preventive therapy (TPT); Ensures high viral load (VL) coverage and implementation of differentiated service delivery models (DSD); Support retention activities such as effective referral through use of referral slips and defaulter identification and tracking; Maintaining HIV/TB programme Monitoring and Evaluation by ensuring the availability and proper use of M&E Tools; Submit daily statistics updates on a shared platform – Official WhatsApp group; Submit weekly and monthly reports of the community post to the Provincial FCI Technical Officer; Submit monthly consumption reports to the ZACH logistics unit and the parent facility Sister-In-Charge; Keep the parent facility Sister-In-Charge informed of all activities during the daily motivational talk at the parent facility; Reviews all data collection tools to ensure completeness of patient’s files and key registers such as Pre-ART, ART, ESTA 4, ESTA 5 and many others and taking the appropriate action.

Qualifications and Experience

Required Qualifications
Minimum of a Registered General Nurse qualification; HIV Rapid testing qualification and experience in OI clinic a distinct advantage; Previous management experience would be an added advantage; Good standing with the community being served is a must; Good knowledge of English and report writing skills required
