About
Bantwana Zimbabwe (BZ) has been implementing the CDC funded through ZimTTECH, Determined, Resilient, Empowered, AIDS-free, Mentored and Safe (DREAMS) program in four high HIV burden districts of Bubi, Lupane, Nkayi and Tsholotsho since October 2020. BZ seeks a suitably qualified Referrals & Linkages Coordinator to close the gaps between referral initiation, completion and linkages to other wrap-around services required to mitigate the HIV / GBV risk of DREAMS AGYW.

Duties and Responsibilities

Roles and responsibilities
Reporting to the DREAMS Programs Manager, the Referrals & Linkages Coordinator will undertake the following duties: Develop, implement, monitor, and document the DREAMS Program Service Layering and Linkage Strategy; Develop the Standard Operating Procedures on the referral pathway and protocol for ensuring that AGYW access services in a timely and appropriate manner; Supervise staff to ensure effectiveness of service delivery including checking for satisfaction of beneficiaries with the timeliness and quality of services; Consolidate all district referral meeting minutes, track trends and gaps weekly and make recommendations for program design; Collaborate with National AIDS Council at provincial level strengthening linkages between social protection and health interventions; Support the analysis of trends of referrals and closure within the DHIS2; Strengthen the link between HIV and GBV by tracking referrals across the indicators; Work with the GBV Coordinator to design and implement a Results Based Financing model for GBV response activities with the ZRP’s Victim Friendly Unit; Provide technical support to case conferencing of at-risk AGYW at ward level with linkages to HIV / GBV response services; Support weekly reports consolidation in liaison with SIE and Programs; Track bi-directional referrals, with accurate reporting weekly.

Qualifications and Experience

Qualifications
A Degree in Social Work, Social Studies, Public Health, Developmental Studies, or any related field; At least 3 years of experience in referrals linkages, tracking or results Based financing; Experience in implementation of community-based programs; Demonstrated ability in project design and implementation of Social Protection programs; Experience in working with different stakeholders, preferably including experience in leading coordination efforts; Excellent interpersonal and communication skills; Ability to work independently, take initiative and leadership.
