Overall responsibility

The District Community Officer is responsible and accountable for implementing and monitoring the OPHID community HIV program activities at district level. The DCO will supervise and work with community level staff and will be responsible for all technical aspects of the program, operational support, and coordination/networking, in line with organizational and national guidelines and targets. The DCO reports to the District Program Coordinator within the district.

Duties and Responsibilities

Main responsibilities

The DCPO is expected to fulfill the following Key Performance Indicators: -

1. Coordinate implementation of community HIV program activities

•Forge enduring relationships with the district community department, district Social Welfare office and other CBOs to harmonize efforts and implementation of HIV programs at community level

•Assist district level structures supporting communities to adopt national HIV strategies and guidelines into their respective implementation plans as relevant

•Participate in strategic community structure meetings at district level and share program updates, progress on implementation plans and challenges that need intervention of relevant offices

•Provide technical support to community structures in adopting a data driven approach to resolving district level challenges affecting the community HIV program

•Support the community structures in coordinating all partners working on HIV programs within the district

•Work with the district level community structures to conduct assessments (including SIMS) and supportive supervision of community HIV programs

•Support the district level community structures in conducting skills audits and coordinating training of community healthcare workers on HIV and TB/HIV programs

•Coordinate and oversee the aggregation, verification, and reporting of all community program daily, weekly, monthly, and quarterly data into OPHID and USAID databases

•Submit timely, accurate and quality reports on all community HIV program activities performed

2. Manage the community HIV program resources for the organization within the district

•Support the DPC to prepare the community program annual budget and workplan for the district

•Oversee the use of organizational resources within the community HIV program in line with OPHID’s policies, the grant agreement, and in compliance with the USAID rules and regulations as applicable

•Manage the performance of all community level staff within the program and ensure that deliverables and outputs are met consistently

•Manage data driven identification of communities, health posts etc, for HRH placement, and the recruitment, orientation, deployment, and performance management of all HRH resources (PCNs, OI/ART Nurses, CXCA Screen, PCNs, PCs, DECs, COAs, COACs, etc) within community service delivery

•Support the DPC to submit monthly projections and monitor expenditure against budget, explaining any variations thereof for all community HIV program activities

•Submit timely requests and acquittals for resources in line with financial management policies and other relevant SOPs to support program implementation

•Support the timely submission of all administrative documents and reports (vehicle logbooks, asset verification checklists, petty cash certificates etc) in line with relevant SOPs

3. Support the district in implementing key HIV prevention activities (including PMTCT) within communities

•Work with the district structures to adopt, implement and monitor national strategies for HIV prevention (, index case testing, HIV self-testing, partner testing, targeted HIV testing) within communities to achieve targets

•Support community structures in implementing awareness and demand generation activities that promote uptake of HIV prevention services

•Support community service delivery platforms to record HIV prevention services correctly and completely in appropriate registers, checklists, and other source documents

•Support community service delivery platforms in identifying gaps and challenges impacting on HIV prevention, and promoting community level QI solutions and/or adoption of best practices from other communities within the district

•Champion community – facility integration in the implementation of all HIV prevention activities

•Promote community level HIV prevention data analysis to improve quality of services and achievement of community targets

•Submit timely, accurate and quality reports on all prevention activities conducted within communities

4. Support the community service delivery teams in enrolling PLHIV onto lifelong ART

•Work with the DHE to adopt, implement and monitor national strategies for enrolling PLHIV onto treatment (same day ART, DTG transition etc) within communities

•Support community service delivery plaforms in implementing appropriate systems (same day ART, hand-to-hand referrals, booking of clients deferring ART initiation etc) that promote uptake and enrolment of PLHIV onto ART within communities

•Support community service delivery platforms to record ART services correctly and completely in appropriate care booklets, registers, checklists, and other source documents

•Support community service delivery platforms in identifying gaps and challenges impacting on ART services, and promoting site level QI solutions and/or adoption of best practices from other communities within the district

•Champion facility – community integration in the implementation of all ART services

•Promote community level ART data analysis (monthly and quarterly cohort linkage analysis) to improve quality of services and achievement of site targets

•Submit timely, accurate and quality report on all ART enrolment activities conducted within the community

5. Support the district and community structures in retaining patients in lifelong ART care

•Work with the DHE to adopt, implement and monitor national strategies for community level retention and monitoring of PLHIV on ART (DSD models, etc) to achieve retention targets

•Support community teams in implementing appropriate systems (DSD models, identification, and referral of patients due for VL, identification and referral of clients with unsuppressed VL) that promote retention and monitoring on ART

•Support community teams in adopting best practices in defaulter tracking within communities to minimize loss to follow up of all clients within the HIV program

•Support community teams to correctly and completely record MMD, VL and defaulter tracing services in appropriate care booklets, registers, checklists, and other source documents

•Support community teams in identifying gaps and challenges impacting on ART retention, and promoting site level QI solutions and/or adoption of best practices from other communities within the district

•Champion facility – community integration in the implementation of all ART retention activities

•Promote community level ART retention data analysis (viral load coverage, viral suppression, remaining on ART, lost to follow up etc) to improve quality of services and achievement of community targets

•Submit timely, accurate and quality reports on all community retention and ART monitoring activities conducted

6. Support the district and community structures in TB and TB/HIV management

•Work with the DHE to adopt, implement and monitor national strategies for community level TB and TB/HIV management (IPT/TPT, GXP testing, TB algorithms etc) at to achieve targets

•Support community teams in implementing appropriate systems (identification of patients eligible for TPT, TB screening, sputum collection, facility linkage etc) that promote scale up of TPT and TB treatment

•Support community teams to record TPT and TB services correctly and completely in appropriate care booklets, registers, and other source documents

•Support community teams in identifying gaps and challenges impacting on TB/HIV, and promoting site level QI solutions and/or adoption of best practices from other sites within the district

•Promote community level TB/HIV data analysis (weekly TPT initiation, TB cascade) to improve quality of services and achievement of community targets

•Submit timely, accurate and quality reports on all TB/HIV activities conducted

7. Support the district and community structures in implementing community cervical cancer screening and treatment

•Work with the DHE to adopt, implement and monitor national strategies for community cervical cancer screening and treatment to achieve targets

•Work with community teams to scale up outreach services for cervical cancer targeting WLHIV on ART in CARGs

•Support community teams in implementing appropriate systems (pre-booking of patients, integrated community ART/cancer screening services, etc) that promote scale up of community cancer screening services

•Support community teams with the deployment of HRH for community CXCA screening and supply chain management of consumables needed for community VIAC activities

•Support community teams to record cervical cancer screening services correctly and completely in appropriate care booklets and registers

•Promote community level cervical cancer screening data analysis (monthly cascade analysis) to improve quality of services and achievement of community targets

•Submit timely reports on all community CXCA screening activities conducted

Qualifications and Experience

Requirements

•Diploma/degree in nursing AND registered as a Registered General Nurse with the Nurses Council of Zimbabwe

•Diploma in community nursing or qualification in project management, health services administration or similar will be an added advantage

•At least 3 years’ experience in clinical management of HIV and TB/HIV at public health facility level

•Completed course in Advanced HIV and AIDS Managed preferred

•At least 5 years’ experience in HIV and TB/HIV programs management at district level within the NGO technical assistance environment or as district community nurse within MOHCC

•Proficiency in MS Office suite (Excel, Word, PowerPoint, Outlook), and online collaboration technologies.
